Prevalence of Infection by Different High-Risk Genotypes of Human Papillomavirus in the Uterine Cervix in Precancerous and Cancerous Lesions

Abstract:

Background: Persistent infection with high-risk human papillomavirus (HR-HPV) is considered a prerequisite infection for development of cervical precancerous lesions and cervical carcinoma Objective: identify human papilloma virus infection by high-risk genotypes prevalence in uterine cervix with CIN 1, CIN2-3 and cervical cancer. Study Design: a prospective study including 2297 women referred by a primary care provider due to abnormal cytology. We selected for the final prospective analysis 1764 patients diagnosed with CIN1, CIN 2-3 and cervical cancer with HPV infection. Prevalence of HR-HPV genotypes in the three groups was analyzed. We used Student's t test for quantitative variables. Comparisons for qualitative variables were analyzed with the chi-squared test. Results: In 538 women more than one HPV were detected (30.5%). The percentage of coinfection varies in the three groups of the study, in CIN1 group 28,5%, in CIN 2-3 group 33.1% and in cervical cancer group 11.5%. HPV 16 was the most frequent genotype detected in the three groups with great difference with the other genotypes being this difference statistically significant (p<0.05). In CIN 1 group the more frequent HPV genotypes were HPV 16(18.2%), HPV 31(6.54%), HPV 51(6.37%) and HPV 66 (5.48%). In CIN 2-3 group the more frequent HPV genotypes were HPV 16 (44.6%) HPV 31(11.33%) HPV 51(6.98%), and HPV 58 (6.79%). In cervical cancer group the more frequent HPV were HPV 16 46.87%, HPV 18 (14.5%), HPV31, 45 and 52 (4.1%). Conclusion: HPV 16 and 18 had more risk of persistent infection and higher risk of progression to cervical cancer.
