Acute Complicated Necrotizing Pancreatitis with Underlying Crohn’s Disease

Abstract:
Acute necrotizing pancreatitis is a complex disease with a high morbidity and mortality rate. In
case of infected necrosis treatment consists of step-up approach involving endoscopic or miniinvasive
surgical methods. In some cases, there are an extremely rare complications would be
presented. In addition, the underlying comorbidities even more worsen the course of the disease.
We are reporting a case of 32-year-old male with acute necrotizing pancreatitis complicated with
recurrent retroperitoneal abscesses, sepsis, iatrogenic pylephlebitis, and exacerbation of underlying
Crohn’s disease and successful outcome of the treatment. During the period of hospitalization one
ultrasound-guided percutaneous drainage, two CT-guided punctures of the retroperitoneal space
(percutaneous and transhepatic) and five Video-Assisted Retroperitoneal Debridement (VARD)
procedures were carried out. The patient was discharged after 185 days of hospitalization.
